The ERAS Token 2027 is the credential international medical graduates need before they can register at MyERAS, submit a residency application, or be visible to any program. ECFMG releases it on June 24, 2026. The ten weeks between Token release and the September 2 application opening are the most strategically important of your cycle. The Token is the binary credential that activates your MyERAS application. Without it, you cannot register, you cannot apply, and your file does not exist in the system that programs read from. One Token covers one ERAS season across unlimited specialties and unlimited programs. Tokens issued for previous seasons do not work. There are no exceptions and no refunds.

Confirm your MyIntealth account is active before June 24
Log in at intealth.org. Identity verification must be complete and your USMLE/ECFMG identification number must be on file. New users should create the account and verify identity in May, not on Token release day.
Confirm your target specialties use ERAS
Emergency Medicine and OB-GYN do not. Those specialties have migrated to ResidencyCAS. The $185 Token fee is non-refundable, so verify before requesting.
Navigate to ERAS Support Services in MyIntealth
From the dashboard, select Services, then ERAS Support Services. You will see an ERAS Token Request tab. The full process is documented at the ECFMG ERAS Application Process page.
Submit the Token request and pay the $185 fee
Payment is made through MyIntealth at the time of request. The request is typically processed the same business day during ECFMG operating hours.
Retrieve the Token from the ERAS Token Request tab
Once issued, the Token remains in your MyIntealth account and can be accessed any time during the ERAS season. Do not let it sit. Move directly to MyERAS registration.
Register at AAMC’s MyERAS portal
Sign in with your AAMC account (or create one). Enter the Token at the AAMC MyERAS registration page, accept the system terms, complete your medical school graduation information, and your MyERAS application becomes active.
Fellowship applicants: different door
If you are currently in a residency program and need a Token for a fellowship application, do not request through MyIntealth. Fellowship Tokens are issued through the ERAS Fellowships Documents Office (EFDO). MyIntealth Tokens are residency-only.
The day your Token is issued is the day program list work begins. Not the week of submission. Not after the personal statement is finished. Now. The program list is the single most consequential strategic decision you will make in the cycle—more consequential than how many programs you apply to, more consequential than how the personal statement is worded.
Programs filter applications in waves. They apply binary filters first: visa sponsorship, year of graduation, ECFMG certification status, USMLE pass. Then quantitative filters: Step 2 CK threshold, USCE hours, research output. Only after these screens do reviewers ever read your file holistically. If your application gets filtered out at any binary stage, the rest of your work is invisible. This is why program list construction is not a volume exercise. It is a fit exercise. For every program you are considering, verify three things in writing on the program’s own website or in published ACGME data. As your list grows, plug your numbers into the ERAS Cost Calculator so the program count never outpaces what you actually intend to spend:
Visa sponsorship
Does the program sponsor J-1? H-1B? Both? Neither? The 2026 NRMP data showed a 13.5-point match-rate gap between permanent-resident IMGs (67.9%) and visa-required IMGs (54.4%). If a program does not sponsor your visa type, your application is filtered out before a human reads it.
Year of graduation cutoff
Many programs publish a maximum years-since-graduation threshold (commonly 3, 5, or 7 years). If you are outside the window, the filter removes you. Cutoffs are usually on the program website or in FREIDA.
Step 2 CK minimum and USCE expectations
Some programs publish a minimum Step 2 CK score. Most require US clinical experience for IMG consideration. Match your profile against the published threshold, not against the program’s prestige.

Register, complete your profile, upload your photo, lock in your medical school graduation information. None of this should be happening in late August.
One page. Specialty-specific. Reviewed by at least two US-based physicians. See the residency personal statement writing guide for the structural anchors.
Four letters. At least three specialty-specific. At least two from US-based supervising physicians where possible. Send formal requests in late June or early July—not August. Writers need 8 to 10 weeks of lead time, and one of those weeks is almost always lost to vacation, hospital coverage, or a delayed reply. Requesting in August leaves you four weeks and a panic. For 2027, letter writers submit through the new AAMC Letter Writer Portal, replacing the old LoR Portal used through ERAS 2026.

ERAS registration does not register you for the Match. NRMP is a separate, mandatory step.
The ERAS Token 2027 request is administratively fast. Everything that has to happen after it—MyERAS registration, document assignment, LoR portal setup, photo upload—takes weeks. Late Token requests almost always produce late submissions.
If your MyIntealth account is not fully verified, the request stalls. Resolve this in May.
A 2026 Token does not work for the 2027 season. The ERAS system rejects prior-season Tokens with no exceptions.
If you are applying only to Emergency Medicine or OB-GYN, you do not need an ERAS Token. Confirm specialty platform before paying $185.
The Token opens the application. It does not build it. Selective screening, holistic review, and goodness-of-fit decisions all happen after submission. The strategic work begins the day the Token is issued.
