What is ECFMG Certification?

ECFMG Certification is required for international medical graduates (IMGs) who want to:

  • Enter a U.S. residency or fellowship program
  • Take USMLE Step 3
  • Get a U.S. medical license

It is the credential that confirms an IMG is ready to enter U.S. graduate medical education. About 25% of all physicians in U.S. graduate medical education and in practice are IMGs — and every one of them holds ECFMG Certification.

Am I an IMG?

You are an IMG if you graduated from a medical school outside the United States — regardless of your citizenship.

Note for Canadian medical school graduates: Individuals who graduate from Canadian medical schools on or after July 1, 2025 are now considered IMGs for the purpose of entering U.S. graduate medical education. Canadian medical school graduates from before July 1, 2025 are not considered IMGs and are not eligible for ECFMG Certification.

01Step One

Check Your Medical School’s Eligibility

Before doing anything else, confirm your medical school is eligible.

  • Go to the World Directory of Medical Schools at www.wdoms.org
  • Look up your medical school
  • Click the “Sponsor Notes” tab
  • Confirm there is an “ECFMG Sponsor Note” that includes your graduation year
  • If there is no Sponsor Note, you are not eligible for ECFMG Certification — stop here

If you believe the absence of a Sponsor Note is an error, contact ECFMG directly at www.ecfmg.org or reach out to your medical school’s administration to ask whether the school has applied for or is in the process of obtaining the ECFMG Sponsor Note.

02Step Two

Create a MyIntealth Account

MyIntealth is the applicant portal where most of your ECFMG work happens.

  • Go to the MyIntealth website
  • Set up your account
  • Complete identity verification (NotaryCam session)

03Step Three

Submit Your Application for ECFMG Certification

  • Log into the MyIntealth Applicant Portal
  • Fill out the application with your medical education details
  • Upload your medical education credentials (diploma, transcripts)

This application confirms your intent to pursue ECFMG Certification and is required before you can be recognized as eligible for the Step exams.

04Step Four

Pass the USMLE Exams Required for ECFMG Certification

You must pass:

  • USMLE Step 1 — Apply through the FSMB website
  • USMLE Step 2 CK — Apply through the FSMB website

Important 2026 procedural change: As of January 12, 2026, IMGs register and pay for all USMLE Step exams through the FSMB portal at usmle.fsmb.org — not MyIntealth. This is the most significant procedural change for IMGs in 2026.

Step 1 became pass/fail in January 2022, which changed how programs read it in IMG applications. 05Step Five

Choose and Apply for an ECFMG Pathway

The ECFMG Pathway is the formal replacement for the discontinued Step 2 CS exam. Every IMG who needs ECFMG Certification must complete a Pathway, unless they already hold a passing Step 2 CS result that is still valid for ECFMG Certification.

  • Visit the Pathways section on the ECFMG website
  • Review the six available Pathways and determine which one you are eligible for
  • Apply for your chosen Pathway through the MyIntealth Applicant Portal
  • Pathways are offered on a seasonal application cycle — pay close attention to application windows

The Pathway application fee is $925 and is non-refundable.

Important — Pathway expiration: Each accepted Pathway has an expiration date based on the season you applied. Your Pathway must be accepted AND unexpired to become ECFMG Certified and to participate in the NRMP Match. Plan your timing carefully.

06Step Six

Pass the OET Medicine Exam

As part of every ECFMG Pathway, you must achieve a satisfactory score on the Occupational English Test (OET) Medicine. This fulfills the communication skills requirement for ECFMG Certification.

  • The OET has four sub-tests: Listening, Reading, Writing, Speaking
  • A satisfactory score requires a minimum of 350 (Grade B) on each sub-test individually
  • Passing three of four does not satisfy the requirement
  • After taking the exam, you must explicitly authorize OET to release your scores to ECFMG through the OET portal — taking the exam is not enough on its own

This OET requirement applies to every Pathway applicant without exception — regardless of native language, citizenship, or the language of medical instruction. There are no waivers.

07Step Seven

Credential Verification

  • ECFMG will contact your medical school directly to verify your final diploma
  • ECFMG also requests that the medical school provide your final medical school transcript
  • You must have completed at least 4 credit years at an eligible school
  • This step can take time and is largely outside your control

Begin credential verification as early as possible. A medical school that takes six months to verify your transcript is not unusual, and that delay does not come out of ECFMG’s processing time — it comes out of your match cycle.

08Step Eight

Receive ECFMG Certification

Once all requirements are verified and met, you will be issued ECFMG Certification.

You can then:

  • Apply to U.S. residency programs through ERAS
  • Take USMLE Step 3
  • Pursue an unrestricted U.S. medical license after completing graduate medical training

How do I begin the ECFMG certification process?
Confirm your medical school is listed in the World Directory of Medical Schools with an ECFMG Sponsor Note covering your graduation year. If yes, apply through the MyIntealth Applicant Portal.
How long does ECFMG certification take?
For most IMGs, 18 to 24 months. Final certification cannot be issued until after you graduate, because ECFMG must verify your final diploma with your medical school.
What does ECFMG certification confirm?
That an IMG is ready to enter U.S. graduate medical education — verified through medical knowledge exams (USMLE Step 1 and USMLE Step 2 CK), demonstrated clinical and communication skills, and primary-source verification of medical education credentials.
Do I need a U.S. visa for residency?
IMGs who are not U.S. citizens or lawful permanent residents need an appropriate visa, most commonly a J-1 sponsored through ECFMG’s Exchange Visitor Sponsorship Program. Visa eligibility is determined case-by-case after a full review of U.S. visa history.
What exams are required?
USMLE Step 1, USMLE Step 2 CK, and the OET Medicine exam (as part of an ECFMG Pathway). Applicants who hold a valid passing Step 2 CS result may use that in place of the Pathway.
Do I need to pass USMLE Step 3 to become ECFMG Certified?
No. Step 3 is not required for ECFMG Certification. In fact, ECFMG Certification is required before an IMG can apply to take Step 3. Step 3 is typically taken during or after the first year of U.S. residency training as a step toward unrestricted U.S. medical licensure.
What credit requirements must I meet?
At least four credit years at a medical school that meets ECFMG requirements. Restrictions apply to transferred credits.
Is the ECFMG Pathway permanent?
Yes. The Pathway program is the permanent replacement for USMLE Step 2 CS, which was discontinued in January 2021.
