If you are an international medical graduate (IMG), you are required to submit a Medical Student Performance Evaluation (MSPE) — often called the Dean’s Letter — as part of your ERAS residency application.

There are two possible ways this document can be submitted:

  • School Upload: If your medical school participates in the MyIntealth Entity Portal, your Dean’s office or registrar uploads the MSPE directly.
  • Applicant Upload: If your school does not participate, you must upload the MSPE yourself through your MyIntealth Applicant Portal.

This article is designed for the second scenario. If your school does not upload the MSPE, here is exactly how to do it yourself.

Step-by-Step: Applicant Upload of MSPE in Intealth

📄 Applicant Upload MSPE Steps

  1. Confirm Responsibility: Ask your Dean’s office if the school uploads MSPEs. If not, you must upload it yourself.
  2. Obtain the Official MSPE: Request the signed and sealed version directly from your medical school.
  3. Prepare the File: Save it as a PDF only, with a clear name such as MSPE_LastName_FirstName.pdf.
  4. Upload in Intealth: Log in → ERAS Support Services → Documents → MSPE Upload → Submit your file.
  5. Verify Status: Ensure it shows as “Received.” Allow up to 5 business days for processing.

⏳ Upload early to avoid delays — programs cannot fully review your ERAS application without the MSPE.

Step 1: Confirm Responsibility

  • Contact your Dean’s office to verify whether your school uses the Intealth Entity Portal.
  • If your school does not, you will see the MSPE Upload option available in your MyIntealth Applicant Portal.

Step 2: Obtain the Official MSPE

  • Request the MSPE from your medical school.
  • Ensure it is the official version, signed and sealed by the school.
  • Personal or altered versions are not accepted.

Step 3: Prepare the File

  • Format must be PDF only.
  • File name should be clear and professional, e.g., MSPE_LastName_FirstName.pdf.
  • Review carefully for completeness before uploading.

Step 4: Upload the Document

  • Log in to your MyIntealth Applicant Portal.
  • Navigate to ERAS Support Services → Documents → MSPE Upload.
  • Select your file and upload.

Step 5: Verify Status

  • Once uploaded, your Intealth portal will show the document as “Received.”
  • Allow up to five business days for processing.
  • If rejected, you will receive instructions to correct and re-submit.

Best Practices for Applicant Upload

  • Do it early — last-minute uploads risk delays in processing.
  • Keep a copy — save the exact PDF you uploaded.
  • Check regularly — monitor your portal until the status shows “Processed.”
  • Avoid errors — unofficial or incomplete MSPEs can hold up your application.

⚠️ Important Note:

A properly submitted MSPE is only useful if reviewed by programs — so it’s critical to ensure not just upload, but also how it’s evaluated. For insight into how the MSPE influences interview selection, see our 2025 MSPE & Residency Interview Selection Guide.

For historical context on how the process worked in ERAS with the Dean’s Letter, see our Older MSPE / Dean’s Letter Article.

Why This Matters

Programs cannot fully evaluate your ERAS application without the MSPE. If your school does not submit it for you, completing the MSPE upload in Intealth yourself — accurately and early — is essential to ensuring your file is ready for program review by October 1.

Important Update (2025):
The MSPE remains a critical component in residency selection. In a survey of Program Directors in primary care, 54.8% rated the MSPE as “very/extremely important” when considering candidates for interviews. (source)

In the 2025 Match cycle, U.S. citizen IMGs had a 67.8% PGY-1 match rate—highlighting how competitive the process is and how every application component matters. (source)

Important Note on October 1: The MSPE is officially released to residency programs on October 1. This is the standard nationwide release date set by ERAS. However, your MSPE must be uploaded and processed in advance (by your school or by you) for it to be included in that release. If the document is uploaded late, it may not be available to programs on October 1 even though your application was submitted earlier.

Final Thoughts

For IMGs, knowing whether the MSPE will be uploaded by your school or by you is a crucial first step. If the responsibility is yours, follow this step-by-step process to avoid unnecessary delays.
