Cleveland Clinic's Internal Medicine Residency is a large academic program anchored at a tertiary/quaternary main campus with a 4+1 (X+Y) block structure that pairs immersive subspecialty inpatient/consult training with longitudinal continuity clinic across 10 community and VA sites in Northeast Ohio. The program's five named tracks (Categorical, Clinician-Educator, Primary Care, VA, and the ABIM Research Pathway) plus internal pathways (Clinical Scholars, Hospital Medicine, AI and Innovation) signal an explicit effort to sort residents by career trajectory rather than offer one generic path.
