Bassett's Transitional Year program is a small (9-resident), rurally-based preliminary-year pathway housed within a community-oriented, integrated rural healthcare network in Cooperstown, NY, serving a culturally and economically diverse but geographically isolated patient population across a 5,600-square-mile service area. The year is structured to give broad-based exposure across medicine and surgery, with 10 weeks of elective time allowing residents to tailor the year toward eligibility credit for advanced specialties such as psychiatry, pathology, anesthesiology, ophthalmology, PM&R, radiation oncology, dermatology, and radiology, or toward categorical credit in IM, family medicine, general surgery, pediatrics, neurology, EM, or OB/GYN. The program frames itself as a bridge year rather than a terminal training identity, emphasizing personalized mentorship, resident wellness/burnout reduction, and a tight-knit lakeside community life rather than research output or subspecialty depth.
