MercyOne North Iowa is a small community-based internal medicine program (six residents per class) in Mason City, Iowa, built around a Catholic-mission health system historically focused on access to care for underserved and rural populations. Clinical training centers on an 18-bed resident-run critical care unit with direct procedural autonomy, supplemented by broad subspecialty exposure (cardiology, nephrology, ID, GI, heme/onc) and a substantial elective block in years two and three. The program frames itself around close resident-faculty relationships and a small-town, low-acuity-of-life lifestyle rather than a research-intensive academic identity, though QI and research opportunities exist. It appears to value residents who thrive in a tight-knit, autonomy-heavy community environment and who are drawn to rural/small-town practice rather than a competitive academic pipeline.
