Mayo Clinic's Anesthesiology Residency in Rochester is a fully integrated, four-year (PGY-1 through CA-3) academic program at a quaternary-care referral center, with residents managing an exceptionally broad case mix, from routine ASA I patients to complex, high-acuity referrals from across the upper Midwest, at a Level 1 trauma center with over 40,000 anesthetics annually across 109+ operating rooms. Research is deeply embedded in departmental identity, with government- and foundation-funded faculty across every subspecialty division, dedicated elective research rotations, and a requirement that every resident present at a national conference, reflecting an expectation of scholarly output rather than mere availability. The signature CA-3 "hallway rotation" structure grants residents faculty-like autonomy over multiple simultaneous complex rooms, and roughly half to two-thirds of graduates pursue fellowship (spanning nearly every ACGME subspecialty), while about one-third enter academic careers and others move into leadership roles in private practice.
