Program Identity & Fit

CHN Family Medicine is a newly established Teaching Health Center program based at a Federally Qualified Health Center in Long Island City, with clinical training extended through Mount Sinai Queens (adult inpatient, ICU, ER, ambulatory specialties) and NYC Health + Hospitals/Elmhurst (OB, pediatric inpatient, safety-net care). The curriculum is built around explicit longitudinal focus areas serving underserved urban populations: HIV care with AAHIVS certification eligibility, gender-affirming care with WPATH GEI certification eligibility, addiction medicine including buprenorphine management, refugee health, POCUS, and chronic disease management, reflecting a full-spectrum ambulatory-first identity rather than a subspecialty-procedural one. Research is present but framed as encouraged collaboration on health-outcomes and operations topics (colorectal screening, blood pressure control, behavioral health) rather than a defining pillar. The program explicitly orients graduates toward becoming primary care leaders, innovators, and advocates within underserved, medically complex urban communities, valuing cultural competency, social-determinants fluency, and team-based holistic care over research output or fellowship subspecialization.
