Program Identity & Fit

This is a rural, community-based Family Medicine residency at Lonesome Pine Hospital in Big Stone Gap, VA, part of the Ballad Health system serving the medically underserved Appalachian coalfield region of Southwest Virginia and Northeast Tennessee. Training is built around longitudinal continuity of care (pediatric, adult, geriatric, and obstetric panels followed across all three years, plus home visits), with inpatient and subspecialty exposure gained through rotations at larger Ballad Health hospitals such as Bristol Regional Medical Center (Level II/III trauma) and Holston Valley Medical Center. The program is explicitly flexible rather than fellowship-driven, allowing residents to tailor training toward hospitalist medicine, outpatient practice, or fellowship (past graduates have pursued EM, geriatrics, and sports medicine), and nearly 70% of graduates remain within the Ballad Health system afterward. It is affiliated with osteopathic medical schools (LMU-DeBusk, University of Pikeville-KYCOM), reflects a DO-friendly clinical culture, and emphasizes community engagement through free clinics, health fairs, and partnerships with local underserved-health organizations like The Health Wagon.
